Doenças hereditárias do metabolismo: importância de um diagnóstico precoce para a criança e para a família
RESUMO: O sucesso do rastreio neonatal da fenilcetonúria nas décadas de 70/80, veio permitir a
partir da década de 90, com o recurso à espectrometria de massa em tandem (MS/MS), a
pesquisa de um grande número de doenças hereditárias do metabolismo (DHM). A
possibilidade de obter um diagnóstico pré-sintomático para estas doenças, que dispõem
de tratamento, é potencialmente gratificante, havendo uma tendência para alargar cada
vez mais o painel das doenças rastreáveis. Assim em Portugal, o Programa Nacional de
Diagnóstico Precoce (PNDP) passou a incluir 24 patologias a partir de 2004.
Presentemente, o benefício do rastreio é inequívoco para um grupo restrito de doenças,
em que se inclui a fenilcetonúria e o défice da desidrogenase dos ácidos gordos de
cadeia média (MCAD), e não consensual para as restantes, com diferentes painéis de
rastreio nos vários países.
Neste trabalho, proposemo-nos avaliar o contributo de um diagnóstico pré-sintomático na
saúde das crianças rastreadas e a importância deste diagnóstico na vida das respectivas
famílias. Este estudo incluiu 253 doentes com DHM, sendo alvo de especial relevância 93
que são afectados pelas patologias recentemente incluídas no painel de rastreio
metabólico neonatal alargado em Portugal. Estes 93 doentes pertencem a dois grupos,
os diagnosticados na fase sintomática com base na apresentação clínica (n=45) e os
rastreados pelo PNDP, na sua maioria em fase pré-sintomática (n=48).
Foram comparados em paralelo os doentes com a mesma DHM em termos de
morbilidade e mortalidade. A fenilcetonúria é a única DHM em que já existem registos
consistentes do seguimento a longo prazo e cujo sucesso serviu de ponto de partida para
o rastreio das restantes doenças. Na avaliação do benefício para o doente foram
especialmente consideradas quatro patologias: leucinose, argininemia, acidúria 3-hidroxi-
3-metilglutárica (3HMG) e défice de MCAD, por serem as mais relevantes na nossa
experiência. Concluímos que o diagnóstico precoce contribuiu para a redução da
morbilidade e da mortalidade aguda e crónica associadas a estas DHM sendo o ganho
para a criança mais efectivo na leucinose, argininemia e défice de MCAD.
No impacto para a família, destaca-se a importância do diagnóstico dos demais membros
afectados, muitas das vezes ainda assintomáticos ou com sintomas minor, mas também
nalgumas situações com quadros incapacitantes e arrastados ao longo dos anos sem
diagnóstico etiológico. Nestes casos, o aconselhamento genético e o diagnóstico prénatal
são uma mais-valia inquestionável.
O rastreio neonatal permite o diagnóstico de todas as variantes da patologia, desde os
casos mais graves aos mais ligeiros, assim como um real conhecimento epidemiológico
das DHM. Estes dados suportam a decisão de incluir no painel português, doenças muito
raras noutros países europeus e que, pelo contrário, são particularmente frequentes entre
nós, nomeadamente a argininemia e a 3HMG.
Uma reflexão sobre o trabalho desenvolvido, permite-nos concluir que a comunicação do
resultado anómalo à família por parte de um clínico experiente no diagnóstico e
tratamento destas patologias, o acompanhamento multidisciplinar, o aconselhamento e
estudo familiar quer a nível bioquímico quer genético e a possibilidade de diagnóstico
pré-natal, fazem desta intervenção integrada na área da pediatria, uma mais-valia para os
doentes do foro metabólico. ABSTRACT: After the success of neonatal screening for phenylketonuria in the 70s/80s and, with the
application in the 90's, of the tandem mass spectrometry (MS/MS), it became possible to
search for a large number of inherited metabolic diseases (IMD). The possibility of having
a early diagnosis for these treatable disorders is potentially rewarding, which lead to a
tendency to extend the panel of screening disorders. Hence, since 2004, in Portugal, the
Programa Nacional de Diagnóstico Precoce (National Neonatal Screening Program)
includes 24 IMD. Currently, the benefit is unequivocal for a limited group of diseases –
including phenylketonuria and medium chain acyl-CoA dehydrogenase deficiency
(MCAD), but the remaining disorders are still subject of discussion, which explains the
existence of heterogeneous screening panels in different countries.
In this study, we proposed ourselves to assess the contribution of the neonatal screening
for the health of children with pre-symptomatic diagnosis and to evaluate the impact of this
diagnosis in the life of their families. This study included 253 metabolic patients, with
particular attention to the 93 patients affected with the diseases recently included in the
Portuguese expanded neonatal metabolic screening panel. These 93 patients were
divided into two groups: those who have been diagnosed in a symptomatic stage (n=45)
and those that have been detected by the neonatal screening program, most of them in
pre-symptomatic stage (n=48).
Within the same IMD, patients of the two groups have been compared for morbidity and
mortality. Phenylketonuria is the only IMD with solid records of long-term follow-up and
whose successful treatment was used as a starting point for the screening of the
remaining diseases. Concerning the evaluation of the benefit for the patient, four diseases
were specifically assessed: MSUD, hyperargininemia, 3-hydroxy-3-methylglutaric aciduria
(3HMG) and MCAD deficiency - because they are the most assumed relevant IMD
according to our experience. We have verified that the early diagnosis contributed to a
reduction of the mortality and morbidity associated with acute and chronic presentations of
these IMD. The gain has been most significant for children with MSUD, hyperargininemia
and MCAD deficiency.
Regarding the impact in the family, we highlight the importance of diagnosing affected
relatives, often still asymptomatic or presenting minor symptoms, but also, in a few cases,
with long time disabling symptoms without adequated diagnosis. In these cases, genetic
counseling and prenatal diagnosis are unquestionably valuable. Expanded neonatal
screening allows for the diagnosis in most of the diseases in its whole spectrum - from
severe to mild – and also for the knowledge of the real epidemiology of the IMD.
These data support the decision of inclusion, in the Portuguese panel, of very rare
diseases in other European countries, which are particularly frequent among us - such as
hyperargininemia and 3HMG.
Some reflection on the work already developed allowed us to conclude that the
communication of abnormal results to the family by an expert clinician in the diagnosis
and treatment of these disorders, the multidisciplinary follow up, the counseling and
familial study (both biochemical and genetic) and the possibility of prenatal diagnosis,
make this integrated intervention in the area of Pediatrics an advantage for metabolic
patients

Risk and resilience in practice: Cultural heritage buildings
[Excerpt] The recent occurrence of devastating natural and man-made hazardous events has raised the awareness ofnumerous international institutions connected to dis-aster risk reduction (DRR) and disaster risk manage-ment (DRM). The development of new risk assessmentand mitigation strategies is, therefore, internationallyencouraged, particularly by focussing on the identifica-tion of the most vulnerable elements to reduce theirvulnerability, while enhancing preparedness and recov-ery capacity. In the particular case of cultural heritage,international frameworks and programmes for DRR areincreasingly echoing concerns about its protection fromdisasters given its irreplaceable value for society.Coherent recommendations for assessing and mitigat-ing disaster risk in the built cultural heritage should,therefore, be considered a priority. O contributo da estética musical na educação : "estudo do Grupo Coral da Freguesia de Esporões"
Dissertação de mestrado em Educação (área de especialização em Educação de Adultos)A presente investigação insere-se no âmbito do Mestrado em Educação, na área de
especialização em Educação de Adultos.
Neste trabalho procuramos compreender o papel da Estética Musical na Educação,
analisando todos os seus contributos para a formação integral das pessoas e da comunidade
onde estas se inserem.
O trabalho está dividido em duas partes. Na Primeira Parte trataremos da
fundamentação teórica e na Segunda Parte abordaremos a investigação empírica.
Assim, abordaremos na Primeira Parte do nosso trabalho os conceitos de Estética e de
Arte, para de seguida nos debruçarmos sobre a evolução da Estética Musical desde a
Antiguidade Clássica até aos nossos dias. Analisaremos as ligações da Estética Musical com os
sentimentos e as emoções, bem como as suas consequências práticas. Este capitulo termina
com uma abordagem sobre a música como forma de comunicação num texto e/ou obra
musical.
A Segunda Parte é dedicada à investigação empírica. Nela faremos a apresentação do
estudo de caso, o Coro Infanto-Juvenil de Esporões, salientando aspetos ligados à sua formação
e à sua origem, bem como a sua ligação e influência na comunidade em que se insere.
Apresentaremos a metodologia, os resultados obtidos e a análise desses resultados.
Procedemos, posteriormente à análise dos resultados obtidos. Estes demonstram, em
nossa opinião, que a ação promovida pelas práticas ligadas à música, exercem sobre as pessoas
influências que podem ser transformadoras e promotoras de autoestima, autoconfiança, sentido
de responsabilidade e de solidariedade.The present research was made within the framework of the Masters in Education, in the
field of specialization in Adult Education.
With this work we aim at understanding the role of Musical Aesthetics in Education by
analyzing all its contributions to the integral formation of individuals and their communities.
The work is divided into two parts. Part One addresses the theoretical background, and
Part II presents the empirical investigation.
Thus, in Part I of our work we approach the concepts of Aesthetics and Art, then we look
into the evolution of Musical Aesthetics, from the classical period to the present day. We shall
analyze the connections between Musical Aesthetics and feelings and emotions, as well as their
practical consequences. This chapter ends with a discussion of music as a form of
communication in a text and / or a musical piece.
Part Two is devoted to the empirical investigation. It begins with the presentation on the
case study - the “Coro Infanto-juvenil de Esporões” (children and youth choir of Esporões) - by
emphasizing aspects related to its formation and origin and its connection to and influence in the
community in which it operates. Methods, results and an analysis of the results are presented.
We then proceed to the analysis of the results, which show, in our opinion, that the
action taken and the practices developed within the context of music can exert a transformative
influence over people, and promote self-esteem, self-confidence, and a sense of responsibility
and solidarity
Cuidados de higiene - baño: significados y perspectivas de los enfermeros
Enquadramento: Como cuidado direto à pessoa, o banho, na sua execução ou supervisão, exalta-se como espaço/tempo de
interação e conhecimento entre enfermeiro/pessoa. Porém, por vezes, é entendido como a tarefa de manter apenas a pessoa
limpa.
Objetivos: Compreender o significado que os enfermeiros atribuem aos cuidados de higiene - banho; identificar os focos,
relativos à pessoa no seu todo, que os enfermeiros consideram aquando da prestação do banho; e descrever o cuidado
desenvolvido durante o banho.
Metodologia: Qualitativa. Estudo descritivo-exploratório, transversal, com recurso à entrevista e observação, com 18 participantes.
Resultados: Orientam-nos para a prestação dos cuidados de higiene - banho, como um cuidado de Enfermagem essencial à
limpeza e conforto do corpo, sendo uma oportunidade para comunicação, presença e relação com a pessoa, onde se pode ter
em conta focos que orientem a prestação de cuidados face às suas necessidades.
Conclusão: Um cuidado que potencia um espaço direcionado à pessoa, que deve ser efetuado pelo enfermeiro, incentivando-a
na participação ativa do seu plano de cuidados.Background: As direct care to the person, the bath, in its
performance or supervision, is emphasised as a space/time of
interaction and knowledge between the nurse and the person.
However, it is sometimes regarded as merely the task of keeping
a person clean.
Objectives: To understand the meaning assigned by nurses to
hygiene care - bath; identify Nursing foci, related to the provision
of Nursing care to the person as a whole, considered during
hygiene care - bath; and describe the care provided during the
bath.
Methodology: Qualitative. A descriptive-exploratory, crosssectional
study was conducted with 18 participants using
interviews and observation.
Results: The results indicate that the provision of health care -
bath is a type of nursing care essential to body cleanliness and
comfort, and offers an opportunity for communication, presence
and relationship with the person. Different foci may be taken into
account to guide the provision of care with a view to meeting the
person’s needs.
Conclusion: A type of care that promotes a space directed to
the person and that must be provided by the nurse, who should
encourage the person to have an active role in the care plan.Antecedentes: Como atención directa a la persona, el
baño, es decir, su ejecución o supervisión, se muestra
como el espacio/tiempo de interacción y conocimiento
entre el enfermero y la persona. Sin embargo, a veces, solo
se considera como la tarea de mantener a la persona limpia.
Objetivos: Comprender el significado que los enfermeros
atribuyen a los cuidados de higiene - baño; identificar los
focos de la persona como un todo que los enfermeros
consideran al dar el baño, y describir el cuidado llevado a
cabo durante el baño.
Metodología: Cualitativa; estudio descriptivo-exploratorio
y transversal en el que se recurrió a la entrevista y la
observación con 18 participantes.
Resultados: Nos orientan a considerar que la prestación
de los cuidados de higiene - baño es una atención de
enfermería esencial para la limpieza y la comodidad del
cuerpo y es una oportunidad para la comunicación, la
presencia y la relación con la persona, en la que se pueden
tener en cuenta focos que orienten la prestación de
cuidados frente a las necesidades.
Conclusión: Una atención que potencia un espacio dirigido
a la persona, que el enfermero debe realizar, fomentando

Maternal Glutaric aciduria type I and newborn screening
Introduction: Expanded Newborn Screening (NBS) programs based on MS/MS, result in a massive
increase of screened metabolic disorders and detected patients. Alongside with the
detection of classical forms of screened metabolic disorders, milder forms (many
whose existence was unknown until now) are being detected, representing major
challenges in respect to follow up protocols.
Disease spectrum of screened disorders is being stretched, through the detection of
undiagnosed mothers through their son’s newborn screening results. In these cases,
abnormal screening results are due to maternal rather to neonatal disease. The most
common examples are newborns from mothers 3-mthylcroyonyl-CoA-carboxylase
deficiency (with newborns usually presenting elevated 3-hydroxy-isovalerylcarnitine
and/or low free carnitine) and from mothers with Primary Carnitine Deficiency (with
newborns usually presenting low free carnitine). More rarely low free carnitne values
in the newborns have been associated to other maternal conditions, namely glutaric
aciduria type I.
Glutaric aciduria type I (GAI) (MIM# 231670) is an autosomal recessive inherited
metabolic caused by a defect of the enzyme glutaryl-CoA dehydrogenase. Clinically
GAI, a neurometabolic disorder firstly described by Goodman is characterised by a
progressive neurodegeneration that typically manifests acutely in infants during a
intercorrent illness. The well known phenotypic presentations are fronto-temporal
brain atrophy with macrocephaly and acute encephalopathic episodes with striatal
necrosis followed by dystonic-dyskinetic movement disorder.info:eu-repo/semantics/publishedVersio
Defeitos congénitos da glicosilação
Congenital disorders of glycosylation are a highly variable, rapidly expanding family of genetic diseases that result from defects in the synthesis of glycans. The vast majority of these monogenic diseases are inherited in an autosomal recessive way, but some types follow an autosomal dominant or X-linked inheritance.
The present work aimed to review the state of the art of congenital disorders of glycosylation, including available therapeutic options, and present a simplified diagnostic approach to this group of diseases.
Congenital disorders of glycosylation can be classified into four categories: N-linked glycosylation defects, O-linked glycosylation defects, combined glycosylation defects, and glycosphingolipid and glycosylphosphatidylinositol anchor synthesis defects. The phenotype may range from mild to severe, depending on disease severity. Clinical features include dysmorphic features, neurologic, dermatologic, cardiac, endocrine, immunologic, hematologic, gastrointestinal and liver involvement, and skeletal muscle abnormalities. As there is no universal or pathognomonic sign or symptom and no sensitive diagnostic test, it is of foremost importance to keep a high index of suspicion of these diseases. When a congenital disorder of glycosylation is suspected, the first step in screening is to perform serum transferrin isoelectric focusing. Molecular genetic testing is the most specific diagnostic test. Treatment is usually symptomatic, with specific treatment only available for some of these disorders.
Since congenital defects of glycosylation may affect any organ at any age and have variable clinical presentation, they should be considered in the differential diagnosis of any patient with multiorgan involvement.info:eu-repo/semantics/publishedVersio
Fígado gordo causado por glicogenose tipo IX: Uma pequena série de casos em crianças
Background: The prevalence of non-alcoholic fatty liver disease (NAFLD) affecting children and adolescents has increased dramatically in recent years. This increase is most probably related to the obesity pandemic and the high consumption of fructose. However, hepatic steatosis has some rare causes (e.g., some metabolic diseases) of which clinicians should be aware, particularly (but not only) when patients are non-obese or non-overweight. Differential diagnosis is notably important when pathologies have a specific treatment, such as for glycogenosis type IX (GSD-IX). Aims: To contribute to the knowledge on the differential diagnosis of NAFLD in paediatric age and to the clinical, biochemical, molecular, and histological characterisations of GSD-IX, a rare metabolic disorder. Methods: We performed a retrospective study of a small series of cases (n = 3) of GSD-IX diagnosed in the past 6 years, who were currently being followed up in the Units of Gastroenterology or Metabolic Diseases of the Paediatric Division of our hospital and whose clinical presentation was NAFLD in paediatric age. Results: Three male patients were diagnosed with NAFLD before 2 years of age, 2 with confirmed diagnosis before the age of 3 years (alanine aminotransferase [ALT], liver ultrasound, and molecular analysis) and 1 whose diagnosis was confirmed at 11 years (ALT, liver ultrasound, liver histology, and molecular analysis). None of the patients were obese or overweight, and the daily fructose consumption was unknown. The outcome was favourable in all 3 patients, with follow-up periods ranging from 2 to 6 years. Conclusion: The decision on how far the search for secondary causes of NAFLD should go can be difficult, and GSD-IX must be on the list of possible causes.Introdução: A prevalência de fígado gordo não-alcoólico (NAFLD) afetando crianças e adolescentes, aumentou enormemente nos últimos anos. Este aumento está muito provavelmente relacionado com a pandemia de obesidade e o elevado consumo de fructose. No entanto, a esteatose hepática pode ser causada por algumas entidades raras (por exemplo, doenças metabólicas) para as quais os clínicos devem estar alertados, particularmente (mas não só) quando os doentes não são obesos nem têm excesso de peso. O diagnóstico diferencial é especialmente importante quando as patologias têm um tratamento específico, como é o caso da Glicogenose tipo IX (GSD-IX). Objetivos: Contribuir para o conhecimento dos diagnósticos diferenciais de NAFLD em idade pediátrica, e contribuir para a caracterização clínica, bioquímica, molecular e histológica da GSD-IX, uma doença metabólica rara. Metodologia:Estudo retrospectivo de uma pequena série de casos (n = 3) de GSD-IX diagnosticados nos últimos 6 anos, atualmente em seguimento nas Unidades de Gastrenterologia ou de Doenças Metabólicas do Serviço de Pediatria do nosso hospital, e cuja forma de apresentação clínica foi NALFLD em idade pediátrica. Resultados: Três doentes de sexo masculino foram diagnosticados com NAFLD antes dos 2 anos de idade, dois com diagnóstico confirmado antes dos 3 anos (alanina aminotransferase [ALT], ecografia hepática, e estudo molecular), e um cujo diagnóstico foi confirmado aos 11 anos (ALT, ecografia e histologia hepáticas, e estudo molecular). Nenhum dos doentes era obeso ou tinha excesso de peso, e o consumo diário de frutose era desconhecido. A evolução foi favorável nos três doentes, com um tempo de seguimento que variouentre 2 e 6 anos. Conclusões: A decisão sobre até onde a pesquisa de causas secundárias de NAFLD deve ir pode ser difícil, e a GSD-IX deve estar na lista de causas possíveis.info:eu-repo/semantics/publishedVersio
